What We Do

Accountable Aging Care Managers work in collaboration with all elder service providers, throughout the continuum of health services and care settings. Although you will most likely experience only a single point of contact with your care manager, know that he or she is backed by a multi-disciplinary team that works together on each client situation. Our combined professional experience and personal backgrounds help us provide competent, compassionate solutions to all eldercare issues.

You and your family are able to come to us for virtually anything you need or when you don't know where to begin. We can assess your situation and recommend the right combination of services for your budget and your personal needs. We handle everything from Elderlife Management, such as navigating government benefits or coordinating the right level of care, to more immediate needs that fall into our "Safety-Net" services, such as monitoring caregiver performance or taking care of bills for an "independent" elder. We charge an hourly rate for all our consulting and management services. Although it is hard to quantify time wasted on government/utility/healthcare/insurance bureaucracies or time saved by having someone else "handle" things that fall into their expertise, our clients regularly tell us an investment in Care Management with Accountable Aging Care Management (AACM) was money well spent.

What is a Care Manager?

"Care management" covers a broad, complex range of services that assist seniors in their day-to-day life choices and activities.

Care Management services can be provided by an individual or company. Care Managers understand and value the eldercare environment. They provide seniors and their families with guidance and resources so they can make smart choices based on their circumstances and family dynamics.

How it Works
Accountable Aging Care Managers can provide a broad spectrum of services and support, tailored to the elder and the family's needs. Some of the solutions could include:
  • Manage caregiver schedules, performance and activities in the home or in a facility
  • Advocate on the patient's behalf with facilities or insurance companies
  • Accompany clients to doctor's appointments
  • Pay bills
  • Ensure compliance with medication prescriptions
